Analy Commuter Form
Car Pool Interest

Fill out the form below and click the "submit" button if you are interested in carpooling to or from Analy High School in the morning or afternoon of the Spring 2009 term. If you are an Analy student, parental permission is required to use this form. Moreover, students under 18 are not allowed to drive students under age 18.

For obvious liability reasons, Analy High School is not responsible for the safety and care of Analy students, drivers or parents in car pools. Drivers and parents must take it upon themselves to make sure that vehicles are in good condition and driven safely, that they are adequately insured, that driving plans are agreed to ahead of time, to have a backup plan, and related matters.

NOTE:  Any information you include on the form below will be made available to other people who choose to download it. Presumably these will be people who are also interested in commuting. Use your common sense. You may leave answers blank for privacy reasons but email address and "where you live" questions may be of great importance for this to work.

The list of interested carpoolers, along with any information they provide, will be made available for downloading on the Analy High School web site. This information will be removed some time after classes begin. The list for downloading will be provided in a spreadsheet form listing any information that you provide below. If you don't want the information made public, do not put it on the form.

Information about carpooling may be announced over Analy News, the school electronic newsletter. It is recommended that you subscribe to Analy News.

To have your information removed from the spreadsheet, email carpooldrop@analyhighschool.org using the email address that you used to enter the information below. This is an all volunteer effort but we will attempt to remove your information within a couple of days.

10. What time do you need to arrive at Analy? (Note: MWF classes begin at 7:35 a.m.; tutorial is T/Th at 7:30 a.m.; regular T/Th classes begin at 8:10)

11. If you drive, how many empty seats will you typically have in your vehicle in the morning?

12. What city/area will you return to in the p.m.?

13. What afternoons do you want to return carpool from Analy? (check all that apply)

14. What time do you plan to leave Analy? (Some involvements meet after school.)

15. If you drive, how many empty seats will you typically have in your vehicle in the afternoon?

16. Write any other preferences or helpful information here, e.g., if you will only accept a ride in a car with other girls or boys.
